I’m Rachel Canavan and I founded Partner Health and Wellness to help patients have access to additional resources that promote their wellness and avoid hospitalization. I am a driven and compassionate Registered Nurse who has been practicing since 2012. I have worked extensively in acute care units, including cardiac telemetry, medical step-down, emergency, procedural, post-acute home health, and vascular access. I thrive on helping patients navigate our healthcare system and enjoying their lives, even while facing burdensome health challenges.
We work with physician offices and their patients to provide Chronic Care Management services. These services are typically covered by Medicare, but sometimes require a copay.
